Alright, I've never made a tutorial with screens before, so now I've decided I am.
First, I'd like to note I'll be using
Destiny952's 99th Brush Set for this tutorial. Best grunge brushes I've seen in a long time.
First, start with a 365x120 layer, transparent.

Fill with WHITE, not black. This will get you a better grunge effect. Make sure your foreground is set to black for color.

Now, before anything, pick your colors. The key to making sure your sig will look nice is seeing it in color as you do it. So, do the color balance as shown.

Alright, now we can begin brushing! Pick a brush from Destiny's 99th, and use it on the
right side of the tag. The brush details should look as if it were going towards the center of the tag.

Repeat this, but from the left side. Again,
make sure the brush details are directed towards the center.

Now, use a brush with vertical details in the center.

You're probably thinking now, "Wow, that was easy!", but we're not quite done yet. Brush some more, alternating between white and black brushes till you get a nice, even background color. There should be some variance, however, and you shouldn't be looking at some mustardy-orange solid background because you overbrushed. This is what I got:

Again, you might be thinking "Now we're done!", but no tag is complete without - that's right - a stock! Pick something that would go with a tag. I'll be using a pre-rendered Dawn of War stock from
InF3cTioN3's photobucket album, and the link to the stock we'll be using is
right here.

(Note: If you have to cut your stock out, just use a polygonal lasso to cut closely around it, then feather 5-10px and duplicate the selection - you should have so little background it won't show up on the tag.)
Now, with your stock window selected, go to Select -> Load Selection... and load "Layer 1 Transparency". Your stock should be selected.

Resize your stock as needed to fit the sig. Help -> Resize Image... -> Online is the easiest way to do this.

Select your resized stock as we did before.

Hit CTRL+C to copy it, then hit CTRL+V to paste it into the sig. Also, use Photoshop's Snap feature to place your stock in the bottom right corner (or if the stock is not cutoff as mine is, place it wherever it is cutoff :P)

Now, we have a problem if you're using my stock. It's too bright where that flame is! But, we can fix this, don't worry. Pick your burn tool.

Set the setting to "Highlights" and start brushing it around the flame area. Try to get what I have here.

We're almost done! Add a border like so. Go Layer -> New -> Layer (forgot to do this first time around XD). Hit CTRL+A first to select your canvas, then go Edit -> Stroke.

(Select -> Modify -> Border)

Almost done! Now, we just have to add some text! Pick either a tech or grunge text. I go to
Dafont.com for mine. I'll use 01 Digitall. Put an outer glow around it.
