Default Roxas/Xion Theory
Well this is gonna be short and to the point, this theory is about Xion and Roxas. So here it goes:

I believe that somehow as long as Xion is with the Organization it affects Roxas but he doesn't know that, I believe that Roxas is disappearing as Xion keeps staying there, because of when Roxas is on Destiny Islands and Riku shows up to talk to Zexion in one of the trailers doesn't it seem that they don't even notice Roxas is there?

What if (since at that time it's CoM) Roxas is disappearing each day and now the Organization believes that Xion is their Number 13?

Now if Roxas IS disappearing because of Xion, could Xion be taking the form of Sora to make her the Nobody of Sora, and could that be why she needs to get to her "Original Self" so that Roxas doesn't disappear and he can still be the Nobody of Sora?

I hope that wasn't too confusing

that scene of zexion and riku not seing roxas seems more like roxas is seing some sort of record in co.

This. They were in CO and I doubt Roxas was their during CoM and Roxas and Riku didnt notice him so it much have been the room replaying the memories that happened in the room.
