Well of course she didn't exist as a part of the organization after KH2.
But I wouldn't say she is dead either, for all we know she could just be data XD
And as for Twilight Thorn, it's just a powerful nobody, some speculate that it's the nobody of a darkside since they are about the same size but that's just a representation of Roxas's past as the Darkside represented Sora's darkside hence the name.

I doubt that's something we will be able to seriously theorize without playing the game. It's too central to the game and we don't have enough information about Xion in the first place.

I don't think it's just a matter of her death. I'm not denying that she dies, but that wouldn't explain anything. I think, for whatever reason, she's mutually forgotten by everyone simultaneously. That would be the only reason to exclude her from nearly everything.
